As announced on September 20, 2013, you can obtain the flu, shingles and pneumonia vaccines from your doctor or from a participating provider listed below. Please note that pharmacies located in Maryland require a doctor’s order or prescription for the shingles and pneumonia vaccines

Be sure to call ahead to ensure your doctor, pharmacy or retail clinic has a supply of the vaccine you need. When you receive your vaccine, you will be responsible for any applicable copayments or deductibles in accordance with your benefit plan.

Other Options
If your coverage allows for out-of-network benefits, you may also receive a vaccine at a non-participating pharmacy or retail clinic. You may be asked to pay the full price of the vaccine at the time of service. You will also need to submit a medical claim form with an itemized receipt listing the cost of the vaccine and the administration fee (if any). Medical claim forms can be found at www.carefirst.com. Click on the Forms link located under the For Members tab.

Check with your physician or primary care provider if you have questions about whether to receive a vaccine, as there may be age and condition restrictions.
