Christian first became aware of the personal support CareFirst offers to members after experiencing a health scare. While at work, he felt faint, collapsed and was taken to the local hospital. He was admitted and underwent numerous tests but the doctors were unable to determine the cause of his episode. Believing it was an unexpected result of a new medication; Christian stopped taking it and didn’t experience any ill effects.
Six months later, Christian’s doctor recommended he try the medication again to help get his high blood pressure under control. Within a day of taking the first dose, he experienced the same issue and ended up back in the hospital. This time, once he was discharged, he received a call from a nurse care coordinator with CareFirst’s Patient-Centered Medical Home (PCMH) program. PCMH offers members who are living with complex or chronic conditions one-on-one support from registered nurses.
Through weekly phone calls with his care coordinator, Christian learned ways to better manage his health conditions which include gastroesophageal reflux disease (GERD), high blood pressure, high cholesterol and sleep apnea. She also arranged a Comprehensive Medication Review, where a dedicated team of pharmacists reviews all the prescription medications a member is taking to help identify potential interactions and confirm the mix of drugs is as effective as possible. Christian felt the medication review was “very valuable and extremely cool.” He recalls, “After they looked at everything I was taking, the dosages, etc. they had a recommendation to replace one of my medications that was giving me side effects I didn’t like with another that works much better.”
Christian’s Care Coordination and Health Coaching Experience
Through this program, trained health coaches provide confidential one-on-one coaching over the phone to help members accomplish their individual health goals like general health management or quitting tobacco.
Once Christian’s health was stable and he was successfully managing his health conditions, he graduated from the PCMH program. However, he soon missed the weekly calls with his care coordinator and the extra support the program provided. He was happy to receive a phone call from CareFirst offering free health coaching. Through this program, trained health coaches provide confidential one-on-one coaching over the phone to help members accomplish their individual health goals like general health management or quitting tobacco.
Christian enrolled in the program and soon began working with his coach, Ashleigh, to stay on track and accomplish additional health goals. Christian says, “Not only is it enjoyable speaking with Ashleigh she is also helping me identify and meet my goals.” Some of these include eating more healthfully, going to the gym and keeping up with preventive doctor appointments.

Since working with Ashleigh, Christian started running again and completed the 10k Bay Bridge Run. He also routinely records his blood pressure and weight measurements.
Ashleigh helped Christian learn how his new CPAP machine works and how it will help with his sleep apnea. With her help, he is faithfully using the machine each night and has learned how to adjust it in order to reach the goals his doctor set.
Christian is a strong supporter of mentoring to help get through life’s challenges and he looks forward to his calls with Ashleigh. He says “I’m a big believer in coaching. Any assistance you can get that helps you with your health is great.”
