Earn from $100–$600 for taking certain steps to get and stay healthy. Your reward can be used toward your annual deductible or out-of-pocket costs like copays or coinsurance related to eligible expenses (medical, prescription drug, dental and vision) under your CareFirst health plan.
You can earn both a participation-based reward and an additional results-based reward. You have 120 days from the date your coverage begins to complete the participation-based reward steps and through the end of your benefit period to earn the results-based reward.

- Select a primary care provider (PCP) who participates in our Patient-Centered Medical Home (PCMH) program—a program that provides your PCP with resources and tools to coordinate all your health care; leading to better health, better communication and better results.
- Consent to receive wellness emails.
- Answer an online health assessment.
- Complete a health screening.

The results of your Health Screening will be measured against certain goals for BMI, blood pressure and blood glucose. Flu vaccine and tobacco status will also be measured.
Don’t worry if your health measures fall outside the desired ranges. You can still earn your results-based reward by working with your PCP to establish a plan to reach your goal. Make sure to allow yourself enough time to make a rescreening appointment with your doctor and to submit your updated results in My Account before the end of your benefit period. If your rescreening results are within the range your doctor established, you will earn the results-based reward.

Once you complete the four steps, most members will receive the incentive in 10-14 days.
If you have a plan with a health savings account (HSA) option, you will typically receive the incentive once you have met the Internal Revenue Service (IRS) minimum deductible for an HSA plan—$1,300 for an individual or $2,600 for a family. In some circumstances, you may be able to receive your incentive right away if certain requirements are met. Log in to My Account to check your eligibility.
